My understanding is that "just about every proposal" is to finance ...

it via payroll taxes just as Medicare is funded now. The collection, monitoring, etc., systems are already in place and would have to be scaled up to accommodate the increased number of clients. The "rich paying for it" idea seems to be another canard thrown out by those favoring the status quo.

Providers now don't have to take Medicare patients. How could you force them to take patients under the new system via a law? The market is much more likely to force providers to take them on. IDK how Medicare handles having providers charge more. I would imagine the new system would simply extend the same provisions to the new clients. Even in countries with true socialized medicine systems, there are doctors whose practices are totally outside the national system, a' la the present "boutique doctors"..
