How does it screw heirs?

If I were king, I would propose letting basis travel and get rid of the estate tax.

I submit death should not be a triggering event for avoiding capital gains (which step-up does). If I give my son or daughter $10,000 of AAPL that I purchased for $1,000, both MV and basis carry. That should be the deal in death transference, too.

I just don't see how receiving something in inheritance where I control the tax recognition (if step up is eliminated) 'screws me'.

To answer your question, the estate tax will not be eliminated. Most likely the exemption will double with step up preserved.
