Newt pushed it really hard. Heritage developed it. And Romney enacted it.
All Republicans, and yes, the key elements of the bill were all the same:
1. Coverage for pre-existing conditions...which requires
2. Community rating (most and least expensive rates relatively close)...and
3. Individual mandates to get healthier people in the pool...which requires
4. Subsidies so that the poor can afford individual mandates (even when healthy) and
5. Penalties (through taxation) to make sure people don't free ride until they need coverage.
If you're not doing single payer, this is almost the only other way to even get close to universal access and affordability. I don't know if there was an employer mandate in the GOP versions, and you can certainly make it work without any employer mandate or role. But the core elements of a non-single payer plan that results in everyone having access to health care that they can afford are all the same.
