First and foremost, you need to increase density in the middle of the city

From there, you need to build your density out. Garden apartments need to become high-rises, townhouses need to become garden apartments, and SFHs need to become townhouses. You also need properties designed for a spectrum of demographic groups so that they are all part of the same community. As part of this process, you have to set aside significant plots for parks because people won't have particularly big yards.

I'm not saying any of this is easy, I'm just saying that it is what needs to be done if you're going to address root causes.
