I've read that, but 20 years ago there was not much


difference in reimibursement rates. The difference now is primarily driven by hospital mergers, creating large hospital chains with increased negotiating leverage in an environment where insurance companies are competing to get hospitals into their provider networm.
In today's private insurance world, refusal by a provider IS an option.
