But my plan for single payer would address both. I'd start with

medicare reimbursement rates and crank them way back. But providers would be free to charge supplementary charges on top. You would have providers that would accept the basic reimbursement rates and optimize around low cost and efficiency, heavy use of NPs and PAs, and you would have providers who would charge a premium (similar to today's concierge practices) for better/quicker access, access to the doctor vs PA/NP, etc.
