I've sort of flipped my view on what single payer should look like.


I used to be of the mindset that we should provide everyone with catastrophic insurance plus cover the cost of routine checkups (to minimize the cost of minor issues becoming major / costly issues if left unchecked) .
I now think we should cover everything (with "elective" exclusions), but just crank the reimbursement rates WAY down (think medicare levels but less). Providers will be free to accept the reimbursement rates or charge a premium on top of the reimbursement rates. I think you will end up with providers specializing in both ends of the market. You will have physicians that set up practices geared around high throughput and efficiency (use of RN's, Nurse Practitioners, and PA's for much of the care) with longer wait times and lower levels of personalized service... but will do so for a very low per patient cost and will operate with just the basic reimbursement rates. Then you will also have physicians that set up practices geared more towards higher levels of personalized service (you actually get to see the doctor vs. just his PA) and shorter wait times, but you will have to pay out of pocket for this additional level of service.

You're a doctor I believe, right? Could something like this work? Medicare for all but with even lower reimbursement rates and with the ability of providers to charge on top of the low reimbursement rates.
