It's pretty easy. Canada has government insurance. UK has government

healthcare, as in the docs and nurses and hospitals work for the UK health service, Switzerland essentially has Obamacare (private insurance exchanges), France and Germany have private health insurance, doctors, nurses and hospitals with a single payer system. There are differences and nuances for each of them and the others, but all of them have universal coverage one way or the other. The US has a patchwork; government healthcare (like the VA), government insurance (like Medicare), private insurance (without single payer which is just bullshit), and of course, no care at all, because universal coverage is socialism or some crap like that.

I can't answer you about California.
