You have a free rider problem with anything close to a public good

and single payer will not be an exception.

this country has been subsidizing the development of rural and small town America for over a century. we pay for a fire and police departments that invest the majority of their time taking care of situations caused by stupid or evil people (though we're clearly paying for protection from those stupid or evil people as well..).

ultimately it comes down to what our society thinks is reasonable for our society to bear. we will clearly tolerate more than a few overweight people, smokers, drinkers, drug users. sure it may piss you off, but there is undoubtedly someone healthier than you out there that would rather not be burdened with the risk of your lifestyle (healthy as it may be). its a game of degrees and incredibly hard to legislate. its just the principle that tends to piss people off.
